CAMBA’s Brooklyn 3D (Drive Down Diabetes) is a diabetes prevention and home self-management program designed to address the widespread prevalence of diabetes and pre-diabetes in low-income communities of color in Brooklyn. The multi-faceted program consists of the following individual, group, and community level components: diabetes self-management education in the home, including personal training sessions; container gardening workshops; and a social marketing campaign. The Personal Training Consultant will coach diabetic and pre-diabetic clients on fitness regimens that align with their diabetes self-management goals.

What the Personal Trainer Does:

  • Provide fitness coaching to 60 diabetic and pre-diabetic clients over the course of one year.
  • Work with nutritional staff to develop fitness plans based on client’s health goals.
  • Provide clients with exercise equipment appropriate for their fitness goals.
  • Refer clients to additional low-cost or free exercise programs in the community.
  • Maintain professional relationships with clients and client confidentiality.
  • Comply with any and all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individually identifiable health information.
  • Adhere to public health requirements during in-person activities, such as social distancing and face coverings.
  • Provide all required information for weekly/monthly/quarterly/annual reports.
  • Create and maintain documentation of all activities.
  • Tasks may be modified, expanded and/or assigned over time.

Minimum Education/Experience Required:

  • Minimum two years’ experience providing personal training services.
  • Nationally recognized Personal Trainer certification preferred.
  • Current CPR/AED/First Aid preferred.
  • Knowledge of diabetes preferred.
  • Excellent customer service skills.

Other Requirements:

  • Bi-lingual Spanish or Haitian Creole preferred.
