CAMBA’s Health Link Program is a Medicaid Health Homes comprehensive care coordination program, servicing clients throughout Brooklyn since 2012. To qualify for Health Home services, clients must be managing two chronic conditions, or be HIV positive, or have a severe mental illness. Our team of Care Coordinators partner with clients and their providers to achieve their individualized goals related to improved health and well-being. While managing physical and mental health and decreasing hospitalizations are priorities in the Health Home program, we take a holistic approach and address all social determinants of health which impact our clients’ lives. Care Coordinators formulate and implement service plans with their clients; monitor progress through regularly scheduled telephone contact and/or face-to-face home, office and hospital visits; and document via service plans, assessments and progress notes.

What the Care Navigator Does:
The Care Navigator supports a team of Care Coordinators and Supervisors in the delivery and documentation of Health Home care management services. Services are provided by phone and in-person office and field work. Activities may include:
- Scheduling appointments for clients with providers and/or their Care Coordinator
- Making reminder calls for appointments and required follow-up to clients
- Escorting clients to appointments
- Facilitating referrals to medical and behavioral health care and social services
- Obtaining documentation from clients, hospitals and providers
- Arranging transportation for clients
- Assisting with Medicaid recertification
- Providing clients with health education
Minimum Education/Experience Required:
- High school diploma or G.E.D. and/or
- Experience working within the Health Care field providing services such as patient outreach, recruitment and enrollment; patient education, and/or care management preferred.
- Bilingual English/Spanish and/or English/Creole preferred.
