More than half of our clients are immigrants and refugees from around the globe. Over 85% of our families are in living in poverty, reflecting the challenges faced by nearly 1.7 million New Yorkers today.

Emergency Transitional Congregate Housing Program 2: CAMBA’s Emergency Transitional Congregate Housing program provides ten units of emergency transitional congregate housing for single men and women who are HIV/AIDS Services Administration (HASA) clients.  HASA is the sole referral source for this housing.  Each individual has their own lockable room but shares common space such as the bathrooms, living room and kitchen.  Clients receive case management and residential/operational services such as meals and assistance when they move to permanent housing.  Case management support is an integral part of achieving the program’s goal of helping clients to attain self-sufficiency.  This includes, but is not limited to, ensuring that clients are connected to and maintain primary health care; assisting them to find and secure permanent housing; and exploring and starting the process of returning to the workforce.  A client stay is limited to six months. However, extensions can be granted by HASA.

What The Case Manager Does:

  • Review all documentation establishing clients’ eligibility for program.
  • Assist clients in completing all CAMBA intake applications and forms.
  • Conduct initial intake/assessment of clients and periodic reassessments.
  • Create and maintain client files containing all required documentation such as service plans, reassessments and minimum weekly case note update.
  • Work with clients to break through barriers to client goals and to assist clients in advocating for themselves and in moving toward self-sufficiency. Recommend and implement strategies to persuade clients to participate more fully in this process.
  • Monitor clients’ progress toward their goals (dates achieved) through regularly scheduled room and office visits, and document via progress notes.
  • Evaluate actual living conditions of clients through regular room inspections of congregate site
  • Recommend closing of cases in which clients have not demonstrated a willingness to participate in Case Management services or have regularly broken the program’s House Rules.
  • Input client data into automated database.
  • Escort clients to appointments (educational, medical, social service, etc.).
  • Follow-up with clients and with referral organizations regarding client contact and progress with referral organization.
  • Serve as a liaison with outside organizations regarding such matters as education, healthcare, housing, social services, legal issues, etc.
  • Assist clients in attaining their Housing related goals by identifying and referring clients to appropriate services both within and outside CAMBA.
  • Conduct housing search efforts for each housing ready client referred (i.e., referrals to housing providers, contact landlords, search for apartment listings, etc.).
  • Develop curriculum to address clients’ Housing and Independent Living related needs including, permanent housing search process and options.
  • Provide individualized counseling on housing related topics.
  • Provide housing related support, training and coordination with the other social services staff.
  • Establish linkages with community-based organizations (i.e., supportive housing programs, etc.) and local landlords.
  • Ensure that quality housing placements are achieved via web-site checks.
  • Obtain and maintain relevant documentation and release forms from clients (i.e., birth certificate, proof of income, medical documents, release of Personal Health Information, etc.).
  • In collaboration with clients, conduct periodic reassessments of plan, including short-term and long-term client goals.
  • Assist clients in advocating for themselves in order to gain housing stability.
  • Recommend and implement strategies to persuade clients to participate fully in the process in order to transition into permanent housing.
  • Provide building desk coverage with oversight of maintenance/security for congregate housing facility.
  • Ensure that vacant rooms are ready for occupancy.
  • Maintain regular communication with funder HASA to ensure that the program reaches maximum occupancy.
  • Provide all required information for weekly/monthly/quarterly/annual reports.
  • Respond immediately to emergencies and inform supervisor.

Minimum Education/Experience Required:

Bachelor’s degree (e. g., B.A., B.S.W.) and four years of applicable experience and/or equivalent experience.

Other Requirements:

  • Knowledge about, understanding of, and ability to work closely with, persons with HIV/AIDS and related issues.

Compensation: -$45,000 annually-
