We reach more than 65,000 individuals and families each year at our 90 locations, including 10,000 youth.  CAMBA serves a diverse cross section of New Yorkers from new mothers in Brownsville to job seekers in the Rockaways. More than half of our clients are immigrants and refugees from around the globe. Over 85% of our families are in living in poverty, reflecting the challenges faced by nearly 1.7 million New Yorkers today.

SDOH Referral Pilot Program: CAMBA is partnering through a contract with New York Presbyterian Hospital (NYPH), to develop a pilot program focused on connecting NYPH patients to services that address a variety of social determinants of health (SDOH). The pilot program will be funded for at least one year and initially consist of a full-time Project Coordinator and Case Manager. Together, the SDOH Referral Team will receive referrals from NYPH’s emergency room, and eventually other units of the hospital. NYPH will refer “risking risk” patients, who have at least one social need and two emergency room visits in the past year. The SDOH Referral Pilot Program will prioritize needs related to food, transportation and housing, but other SDOH may be identified and addressed. CAMBA and NYPH will work closely to develop an efficient closed-loop referral workflow and reporting structure.

What The Program Manager Does:

  • Maintain professional relationships with clients and client confidentiality.
  • Practice Universal Precautions/Standard Protocol & Procedures.
  • Comply with any and all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individually identifiable health information.
  • Understand and remain up-to-date on all CAMBA programs and services that may be beneficial to NYPH patients.
  • Research external community programs and services that are complimentary to CAMBA programs and develop relationships with community partners.
  • Respond by phone to NYPH patient referrals within 48 hours.
  • Educate eligible individuals about service offerings and health benefits of the program.
  • Support completion of intake paperwork and comprehensive assessments with patients who consent to the program. This may be by phone, video, email, mail, or in-person.
  • Provide short-term case management services for approximately 1-2 months, with at least weekly follow-up.
  • Enter client data into CAMBA database within 24 hours (i.e. contact information, assessments, client services, progress, outcomes, etc.)
  • Work with clients to break through barriers to their goals and to assist them in advocating for themselves and in moving toward self-sufficiency.
  • Assist clients in completing applications for benefits and entitlements (e.g. Medicaid, etc.), and process applications on clients’ behalf.
  • Monitor clients’ progress towards their service goals, and document all interactions with clients via progress notes.
  • Create and maintain client files and make copies of all client documents.
  • Collaborate with other CAMBA programs, community partners and NYPH team members (such as: patient navigators, community navigators, social workers, care managers, and care coordinators) to improve patient outcomes.
  • Participate in all required SDOH Pilot Program meetings with CAMBA and NYPH leadership.
  • Participate in outreach, planning and execution of special events, including workshops, health fairs, and jointly agreed upon community events.
  • Lead, train and directly supervise a case manager(s) according to CAMBA’s Supervisor Manual policies and procedures, including but not limited to: daily oversight, weekly supervision, annual performance evaluations, professional development and corrective action as needed.
  • Ensure all direct reporting staff completes all necessary case management services and documentation according to program policy.
  • Conduct monthly chart reviews to ensure program compliance and client progress. Provide feedback and support to staff as well as follow-up on all charts reviewed to verify corrections have been made.
  • Develop and implement quality assurance practices and make recommendations for quality improvements across the program.
  • Provide all required information for weekly/monthly/quarterly/annual reports.
  • Assist with the development of program content, policies and procedures to address client needs related to housing, food, transportation and other social determinants of health.
  • Troubleshoot client and direct reporting staff conflicts and make decisions in accordance with program policies, procedures, and protocols.
  • May follow-up with clients for a period of time after successful completion of their primary goals to assure client stability.
  • Tasks may be modified, expanded and/or assigned over time.

 

MINIMUM EDUCATION/EXPERIENCE REQUIRED:

  • Bachelor’s Degree (BA or BS) and at least four years of applicable experience, and/or equivalent experience. Licensed Master’s Degree in Social Work (LMSW or LCSW), Licensed Mental Health Counselor (LMHC), Master’s in Public Health, or Master’s in Human Services, preferred.

 

OTHER REQUIREMENTS:

  • At least 2 years case management or direct client care experience, required.
  • At least 2 years supervisory and/or quality assurance experience, required.
  • Bi-lingual English/Spanish and/or English/Creole speakers, strongly preferred.
  • Knowledge about, understanding of, and ability to work closely with, persons with chronic illness and a variety of needs related to the Social Determinants of Health.
  • Knowledge of community resources and sensitivity toward persons with chronic illnesses and multiple social needs.
  • Follow all HIPAA privacy rules as they relate to personal health information and documentation.
  • Ability to utilize public transportation to navigate the five boroughs of New York City.
  • Ability to utilize mobile technology as a tablet to document while performing field work.

 

Compensation: $60,000-$65,000 annually
Status: Full-Time (35 hours per week), some flexibility with hybrid schedule of office and remote work.
