iCare Program

iCare works with women before, during, and after pregnancy to empower them regarding health care and reproductive health decision-making. iCare Community Health Workers provide case management services to connect women to quality health care and other resources to support their health and well-being. CHWs also conduct educational workshops and facilitate supportive groups. Services are provided through monthly home visits and office sessions.

iCare is available to work with women residing in the following zip codes: 11226, 11203, 11233, and 11212.

iCare is funded by the NY State Department of Health.

Eligibility: Women of childbearing age (ages 12-44), including teenagers, pregnant women, and mothers of young children living in the following Brooklyn zip codes: 11226, 11203, 11233, 11212
